FAMILY AND COUPLE COVERAGE

Each year you are given one opportunity to purchase family/couple coverage. All completed forms and applicable fees must be received no later than 30 days from the start date of your benefits program. E.g. If your coverage started September 1, 2005, the deadline for adding family/couple coverage would be September 30th, 2005.

Family add-on fees are non-refundable.

Family and Couple coverage rates are in addition to your single semesterly rate.

Family/Couple add on's do not carry forward and must be renewed annually.

Family/Couple rates are annual.

You can add dental AND/or extended health.

Rates effective September 2008 to August 31st 2009

Couple Coverage:

$292.80 Dental
$173.16 Extended Health
$465.96 For both dental and extended health.

Family Coverage:

$465.60 Dental
$266.16 Extended Health
$731.76 For both dental and extended health

Note:

- Couple coverage is for adding a spouse OR 1 child
- Family coverage is for adding a spouse and child(ren)

We do not currently accept cash, credit cards, interac or personal cheques. We currently accept certified cheques or money orders only.

Please follow these steps to add family or couple coverage:

Step 1
Determine when your deadline is. If you unsure and require assistance, please contact the Benefits Office.

Step 2
Assuming it is prior to the deadline, you would complete a "Family Add On Form".

Step 3
Obtain a certified cheque or money order for the applicable amount, made payable to : The Graduate Student Society at SFU

Step 4
Return the completed Family Add on Form along with your payment to the Benefit Plan Office via mail or in person before the deadline.

Upon receipt of your form and payment we will issue you a receipt.
