OPTING OUT
PLEASE READ EVERYTHING ON THIS PAGE AS IT IS VITAL INFORMATION.
Do I have to opt out each semester?
No. As it is a one time only opt out, you only need to opt out ONCE
and the opt out will remain in effect for the duration of your studies at SFU.
Students who have comparable existing coverage through another insurance company can choose to coordinate benefits or waive their participation in the Grad Benefit Plan by opting out using the online form below.
If you have existing extended health and/or dental coverage you can also choose to coordinate this coverage with your Grad Benefit Plan coverage instead of opting out.
How do I Coordinate my Benefits?
Step 1) Expenses are first submitted to your primary coverage with a complete claim form.
Step 2) When you receive payment from your primary coverage, you would then submit the same receipts or copies of the receipts to your secondary coverage along with a completed claim form and the "explanation of benefits" from the primary coverage. (The explanation of benefits shows the amount and description of what the insurance company has paid)
How do I Opt Out?
**IMPORTANT**
To opt out you must provide documentation of your existing dental and/or extended health coverage.
If you have your proof of coverage as a paper document, you can scan those items using a scanner or take a clear picture of your documentation using a digital camera. Only .pdf, .jpg, .png, .gif, .doc and .tif files will be accepted.
Examples of documentation include:
- A photocopy of a certificate or card clearly indicating your name, your coverage for extended health and/or dental care, the insurance company name, and the policy number, OR
- A letter from the plan sponsor (usually the employer) or the insurance company attesting that you’re covered for supplemental health and/or dental care.
- Aboriginal students who receive health and/or dental benefits from Health Canada may use their status card as proof of other coverage.
MSP or other provincial basic medical coverage is NOT considered comparable coverage and CANNOT be use to opt out.
Opt outs submitted without documentation will not be accepted.
When are opt outs processed?
Opt outs are not "instantaneous".
- September opt outs are processed by the end of October
- January opt outs are processed by the end of February
- May opt outs are processed by the end of June
When you complete the online opt out, you will receive an automatic email confirmation which confirms that we have received your submission.
Make sure you use an email address where you can rectrieve emails and where the email receipt will not be filtered in to the "junk mail" box.
Out Out Deadlines
You must opt-out/waive benefits no later than 30 days from the start date of your benefits program.
Your benefit program starts on the first day of the month for the semester you start your initial Graduate classes/enroll in.
You may only opt out/in once per year on the anniversary date of your coverage. If you are not sure when that is, contact the Benefit Plan Office.
Semesters Deadlines
September September 30 - 4:00 p.m.
January January 30 - 4:00 p.m.
May May 30 - 4:00 p.m.
